ESTRO, ACROP and SIOPE Recommendations for Myeloablative Total Body Irradiation in Children

Myeloablative Total Body Irradiation (TBI) is an important modality in conditioning for allogeneic hematopoietic stem cell transplantation (HSCT), especially in children with high-risk acute lymphoblastic leukaemia (ALL).

Myeloablative Total Body Irradiation (TBI) is an important modality in conditioning for allogeneic hematopoietic stem cell transplantation (HSCT), especially in children with high-risk acute lymphoblastic leukaemia (ALL). TBI practices are heterogeneous and institution-specific. Since TBI is associated with multiple late adverse effects, recommendations may help to standardize practices and improve the outcome versus toxicity ratio for children.

The European Society for Paediatric Oncology (SIOPE) Radiotherapy TBI Working Group together with ESTRO experts conducted a literature search and evaluation regarding myeloablative TBI techniques and toxicities in children. Findings were discussed in bimonthly virtual meetings and consensus recommendations were established.

The aim of this clinical practice guideline (CPG) was to harmonize international recommendations for mental health surveillance in survivors of CAYA cancer diagnosed prior to age 25 years. This CPG was developed by a multidisciplinary panel of experts under the sponsorship of the International Guideline Harmonization Group (IGHG). The experts evaluated concordance amongst existing survivorship CPGs and conducted a systematic review following evidence-based methods. Of 7,249 studies identified, 76 articles from 12 countries met inclusion criteria. Recommendations were formulated based on identified evidence in combination with clinical considerations. This international CPG strongly recommends mental health surveillance for all survivors of CAYA cancers at every follow-up visit and prompt referral to mental health specialists when problems are identified. Overall, the recommendations reflect the necessity of mental health surveillance as part of comprehensive survivor-focused healthcare.
