At Hospital Sant Joan de Déu there is a Transition Area which, together with the professionals, has prepared this document to explain the key points of the process so that you can find out how they will accompany you along the way hand in hand with the A10 Transition Program.

In the event that you are the mother, father or guardian of a person with functional diversity, this guide will also help you prepare for your child’s transition to the adult system.
