This process is a companion to the ongoing medical monitoring and late effects management of survivors.

The psychosocial survivorship screening tool and care plan are designed to be implemented as part of a holistic multidisciplinary approach to AYA survivorship care.

They are not to be used as a substitute for clinical judgment or multidisciplinary team planning.
